  • Under the Polish law, the shipment of human ashes to Poland is possible by obtaining a transportation permit issued by a Polish consul.


    In order to obtain the permit you must submit to this office the following:


    • written permission for shipment and burial of human ashes issued by an appropriate County Executive (Starosta, Mayor) in Poland. The Embassy may assist you in obtaining it upon receiving a fax or email copy of the Death Certificate, name and address of cemetery in Poland where the human ashes will be buried and the name and address of the person responsible for burial in Poland;
    • New Zealand death certificate with Apostille attached;
    • Certificate of Cremation;
    • Burial Transit Permit for removal out of New Zealand;
    • Affidavit by funeral Director, stating that the ashes were placed in the hermetically sealed metal urn or similar container, and there is nothing but the ashes inside;
    • Consular fee (cheque payable to the "Embassy of Poland" or cash). The fee covers a permit and an official translation into Polish of the New Zealand Death Certificate.

    Also, one must supply the name of the person or the company responsible for shipment (e.g. a family member, an airline, etc.). A permit will be issued when all the necessary documents are received. Also NZ Death Certificate and its official translation into Polish will be send to you along with the permit.
