    on requirements for shipment of human ashes to Poland


    Under Polish Law, the shipment of human ashes to Poland is possible by obtaining a transportation permit issued by an appropriate Polish consular office.


    Attention: Effective August 1st, 2007, the consular offices will cease sealing the urns.


    In order to obtain the permit you must submit to this office the following:


    • Written permission for shipment and burial of human ashes issued by an appropriate County Executive (Starosta, Mayor) in Poland - a fax copy is acceptable;
    • Death Certificate (original or certified copy);
    • Certificate of Cremation;
    • Notarized affidavit by funeral Director, stating that the ashes were placed in the  sealed urn or similar container, and there is nothing but the ashes inside;
    • Consular fee of $60 (Money Order payable to the "Embassy of Poland")



            * In order to transport the human ashes beyond states such as: Alabama, District of Columbia, Florida, Georgia, Kentucky, Maryland, North Carolina, South Carolina, Tennessee, Virginia, West Virginia, Puerto Rico, American Virgin Islands, Commonwealth of the Bahamas, Guam, Commonwealth of the Northern Mariana Islands, you must also obtain a Burial Transit Permit  for removal out of State issued by the local registrar.

           * The above-mentioned documents should be accompanied by a return envelope with a tracking numer.


    One must supply the name of the person or the company responsible for shipment (e.g. a family member, an airline, etc.). A permit will be issued when all the necessary documents are received.
