    GENERAL INFORMATION

     

     Polish passport is a document which entitles its owner to cross the border and stay abroad. It certifies Polish citizenship as well as the identity of the owner within the scope of the personal data included.

     

     Polish passport can only be issued to a Polish citizen whose identity can be proven on the basis of the documents presented.

     

    Passports are printed in Poland, regardless of the residence of the applicant.  A Polish citizen who is applying for a Polish passport abroad should take into account the fact that the process will last around 8 weeks due to the fact that the document has to be produced in Poland and shipped abroad. It is recommended that an application for a new passport is submitted before the expiration date of the current passport.

     

    Passport for a person above 13 years old is valid for 10 years from the date of issue. A minor under 13 obtains a passport valid for 5 years

     

    In order to schedule an appointment to apply for a passport, it is necessary to register on the website:

     www.e-konsulat.gov.pl

     

    Only registered applicants can be received.

     

    MAKING APPOINTMENTS

     

    In order to submit your passport application, please go to the website www.e-konsulat.gov.pl

     

     

     

    PLEASE BEAR IN MIND THE FOLLOWING:

     

    1. Only registered applicants can be received. While registering you will be able to choose date and time of your visit. The number of registered applicants for each day is limited.

     

    2. In case you cannot show up at the Consulate please do cancel your appointment so that other applicants can use your spot. You can do it via the website, by sending an e-mail to waszyngton.amb.wk@msz.gov.pl

     

    3. Please register in Polish. Multiple registrations of the same applicant will be annulled.

     

    4. Please register only if you have all the documents, i.e.: confirmation of the Polish citizenship, Polish birth and marriage certificates, etc. If you miss them, please check the section: PASSPORT FOR AN ADULT/MINOR. If parents register their child’s application they don’t need to register themselves, as they only accompany their child.

     

    5. Please leave your telephone number. This will speed up the processing of your application and enable us to contact you in advance if needed.

     

    6. If you resign from the visit at the Consulate, you can only come again after another registration is made.

     

    Please arrive on time (10-15 minutes prior to your appointment).

     

    Registering your appointment: step by step

     

    For sections 1-4 you can be directed in English after choosing language in the upper right corner. Further registration is carried out in Polish.

     

    1.Go to the website: www.e-konsulat.gov.pl,

    2.Choose a country: USA,

    3.Choose a diplomatic mission: Washington,

    4.On the left side, choose: Visit – passport,

    5.In the first slot - choose the number of applicants (“I want to book an appointment for:” means "Chcę zarezerwować termin dla" - choose a number of people),

    6.Set a date for the appointment (“Zarezerwuj termin wizyty”),

    7.Write your contact details: first name, last name, address, phone number, e-mail, description (not obligatory),

    8.Choose a type of passport,

    9.Write personal details of applicant/s. (PESEL obligatory only if you have one, if you do not have the PESEL number please write "BRAK NUMERU PESEL"),

    10.Continue (Dalej),

    11.Submit (Zapisz),

    12.Download confirmation (Pobierz potwierdzenie wizyty).

     

    How to cancel your appointment

     

    1.Go to the following website: www.e-konsulat.gov.pl,

    2.Choose a country: USA,

    3.Choose a diplomatic mission: Washington,

    4.On the left side, choose: "cancel appointment”,

    5.Fill in the cancelation form with the number from your registration form and the code received during making registration,

    6.Choose: "Odwołaj wizytę",

    7.Submit (“Zapisz”),

    8.Download confirmation („Pobierz potwierdzenie anulowania wizyty”).

     

    PASSPORT FOR AN ADULT

     

    The application for the passport with all the documents must be submitted IN PERSON and there are no exceptions to this regulation.

     

    A person applying for a passport submits following documents:

     

    1. Passport application filled out in Polish according to our instructions.

      The application must be printed double sided on one sheet of paper.
       
    2. A current (taken in the past 6 months) passport photo sized 35mm x 45mm taken of the face looking directly forward with a natural expression, eyes must be open, eyebrows unveiled and  mouth closed, without a head cover and without dark glasses; the face should constitute 70-80 % of the photo’s surface.
       
    3. If a child does not have a PESEL number an original of a Polish birth certificate must be submitted. In order to receive the Polish birth certificate of a minor born in USA or in a different country SEE. POLISH BIRTH, MARRIAGE, DEATH CERTIFICATES. REGISTRATION OF FOREIGN CERTIFICATES IN POLAND.
       
    4. Documents proving the eligibility for the reduction of the consular fee (for example student ID).
       
    5. Other documents, e.g.: document with the photo certifying the identity of the child and the parents.

     

     

     

    Additional information:

     

    Please be aware that you may be requested to submit other documents such as: birth and marriage certificates, or documents proving Polish citizenship.

    While applying for a passport you will be asked to present your current Polish passport. In case of loss of or damage to the passport, you need to submit official statement along with the application for the passport.

     Polish passport is issued exclusively on the basis of Polish Registry Office’s documents. This means that people who have birth or marriage certificate issued by a foreign Registry Office, for example American one, should transcript these documents.     Polish passport contains biometric data: photo and fingerprints.

     

    PASSPORT FOR A MINOR

     

    Passports for minors under 13 years old are issued for 5 years.

    Passports for minors above 13 years old are issued for 10 years.

    Both parents must be present when submitting the application for a passport on behalf of their child, unless one of the parents has been deprived, by the decision of a Polish court, of his/her parental authority or this authority has been limited. In case there is no consent between the parents or when it is not possible to obtain consent from one of them, a Polish family court's decision replaces that consent for issuing a passport.  Judgments of foreign courts that settle the above issues can only be considered when they have been recognized by relevant Polish courts. 

     

    In the event of the death of one of the parents, it is necessary to provide an original death certificate with apostille.

     

    An application submitted by one of the parents together with a written consent of the other parent with his/her signature that has been confirmed by a Notary Public or by a passport organ is considered as if it had been submitted by both parents. The child's parent who is submitting a passport application should show his/her valid Polish passport. 

     

    A child over 5 years old must be present when submitting the application.

    • While applying for a passport for a minor, parents should present their passports.
    • Parents who do not apply for their passports and only accompany the minor should not be registered in the system.
    • All the documents and fee must be submitted at the moment of application.

       

      TEMPORARY PASSPORT

       

      A temporary passport can be issued:

    • to enable a return trip to the place of residence in case of a lost/stolen documents or for the pending period for the 10-year passport (or a 5-year one in case of a minor),
    • in extraordinary cases as a first passport for a minor or when fingerprints can’t be taken due to a temporary obstacle,
    • in other justified and documented extraordinary cases, such as unexpected health, family, business cases.

       

      Validity of a temporary passport is up to 12 months.

      PERSONAL DATA CHANGE

       

      In case of changing the data of the person who already holds valid Polish passport (for example after changing the last name due to marriage), the person residing abroad is obliged to apply for a new passport no later than 90 days after the marriage or administrative decision about changing the name or decision of a Polish court has been delivered. 

       

       Passport will expire 4 months afterwards.

       

      COLLECTION OF PASSPORTS

       

      Passports have to be collected personally. It is not possible to collect them via third persons.

      Persons who wish to collect their Polish passport do not need to register on the Internet. 

       

      In order to collect your Polish passport, please present:

    1. (Previous) Polish passport for cancellation,
    2. If it is your first Polish passport – document proving identity (US or another foreign passport).

     

    Passports for minors can be collected by one of the parents.

     

    It is possible to send passport by regular mail only in extraordinary and justified circumstances on a special written request of the applicant who must prove their extraordinary circumstances.

     

    FEES

     

    Consular fees are based on the current exchange rates of EUR to USD.

    Attention: Fees are accepted only in cash or money order issued to the Embassy of Poland in Washington, DC. Neither credit cards nor personal checks are accepted.

    The consular fees must be submitted along with the application.

    Current passport fee for a passport for 10-years for an adult is 132 USD.

     

    Persons entitled to 50 % fee reduction:

     

    • pupils and students - a valid student card must be presented or an official declaration of attendance from school signed by a school official and with the stamp of the school, where the given person studies full-time (does not apply for a person under 18 years old).

    • pensioners and veterans who present a proper and valid card or pension receipt.

    Persons entitled to passports free of charge:

    • applicants at the age of 70 years old and more (considering the date of the submission),

    • residents of social assistance houses.

     

    Persons entitled to reduced fees applying for a new passport before the expiry date of the previous one by one tenth of the consular charge for each year before the expiry date in consideration of:

     

    • change of the name or other data written in the passport (after submitting administrative decision or marriage certificate),

    • change of appearance,

    • lack of free pages for visas or border-crossing stamps.

     

    Higher fee - 200% of the regular fees applies to:

     

    • applicants who lost their passport or their passport was damaged and the owner of the passport bears the full responsibility for the loss or damage.
